Position SummaryAt Stony Brook Medicine, the Patient Navigator serves as an initial point of contact for patients and their families to direct and facilitate their cancer care experience. Navigators guide patients, facilitate access to services and patient-health care provider relationships, and coordinate care to assure timely and effective transitions during the patient's course of diagnosis and treatment.Duties of a Patient Navigator may include the following but are not limited to:* New patient intake - helps accelerate evaluations of new patients and facilitates coordination of patient testing and care scheduling.* Serve as a resource during the patient's transitions of care to facilitate timely access to services and care.* Facilitate communication between and among the patient and providers, and act as a liaison with the health care team on behalf of the patient.* Act as the primary point of contact for newly diagnosed cancer patients.* Initiate contact with patients before their initial Cancer Center appointment.* Schedule appointments and collect patient information, including demographics, insurance details, and clinical data.* Verify insurance, obtain referrals, and address potential financial concerns.* Assist patients in obtaining necessary medical reports and records.* Meet with patients at their initial appointment, offering support, explaining the care plan, and coordinating appointments and consultations.* Identify logistical barriers and social needs (social determinants of health) affecting patients.* Assess and address the needs of patients, caregivers, and families, providing informational materials and community resources.* Collaborate with the healthcare team to design and coordinate a patient-centered care plan.* Identify patients with a cancer diagnosis who require navigator services.* Document pertinent information in the electronic medical record.* Expedite appointment scheduling for consultations and support services.* Serve as a patient advocate within the multidisciplinary team.* Provide emotional and educational support to patients and families.* Assist in patient clinics with the multidisciplinary team.* Handle and document patient and caregiver phone calls.* Manage databases, track patient follow-ups, and ensure adherence to recommended cancer care intervals.* Contribute to continuous quality improvement efforts to meet national standards.* Pursue educational opportunities and maintain certifications.* Collaborate with the cancer center's community outreach program director(s) for community health eventQualificationsRequired Qualifications: Bachelor's degree and at least 2 years of experience working with cancer patient population.Preferred Qualifications: Oncology Patient Navigator-Certified Generalist (OPN-CG) certification.Special Notes: Resume/CV should be included with the online application.Posting Overview: This position will remain posted until filled or for a maximum of 90 days. The specific salary offer will be based on the candidate's validated years of comparable experience.
