Patient Navigator I - Tarzana : Job Details

Seeking full time Patient Navigator for program that will serve homeless adults enrolled in Project Room key . The Patient Navigator will be co-located in the office and Project Room key location and work collaboratively with other community partners to help navigate clients through a variety of services. . The ideal candidate will have strong oral and written communication skills and provide excellent customer service that is sensitive to the challenges that homeless persons with a range of medical and behavioral health issues face. Duties will include, conducting behavioral assessments; service planning; linkages to health, mental health, substance use disorder services and other supportive services; crisis management; client education; and housing location services.

Requirements

* BA/BS in Social Work, Psychology or related social sciences field.
