Position Summary:

We're looking for a committed, professional full-time Patient Navigator for our Manhattan location. The ideal candidate is someone who is personable, works well with a diverse population, able to communicate effectively well with medical providers and support staff and has good attention to detail. The Patient Navigator provides field-based case management services to patients enrolled in the HIV care coordination program. Patient Navigator is the main point of contact for the patient. The Patient Navigator builds strong relationships with HIV positive patient in order to help them connected and to stay engage in prevention and treatment services. Patient Navigator is committed to removing the patients barrier to care by identifying critical resources and helping them navigate through health care services and system and to promote patients health. Patient Navigator works closely with the care team which may include doctors, nurses, and other staff in the care team to ensure the patients optimal health care outcomes.

Job Responsibilities:

+ Provide direct services to person who living with HIV/AIDS under supervision of Care Coordinator.

+ Carries out tasks that are needed to execute the medical and support service plans, including the following: accompanies patients to appointments when required, provides coaching to patients, delivers monthly or weekly health education encounters, performs entitlements reassessment, coordinates logistics for plan adherence reminders, transportation and childcare arrangement.

+ Responsible for field-based following tasks: observes and records patient self-administration of ARVs., assess for and reports any ARV related side-effects, and delivered in the home, CBO, work or any other location that is convenient for PLWHA.

+ Maintain client-related records and other required documentation according to the protocols and standards of APICHAs Client Services Department.

+ Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks.

+ Represent the agency at various venues to promote agency services.

+ Other activities as assigned.

Qualifications:

+ Associates degree in human services or social work and at least 12 months experience in patient navigation or similar field.

+ May consider certification as a peer worker by the NYS AIDS Institute Peer Certification, or case management certification by CUCS, or at least 90 hours of documented training equivalent to those required by the NYS Peer Worker.

+ Strong socio-cultural identification with the target population or experience in working with target population required.

+ Fluency in one or more Asian language preferred.

+ Excellent written and verbal communication skills in English required.

+ Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season) and testing for tuberculosis is required. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
