Position Summary:The Patient Navigator will support patients and their families in accessing needed services including registration, scheduling, and general appointment services for their New York Proton Center visit. Additional responsibilities for this role include making phone calls to appropriate agencies to advocate for services and following up on the patient's status. The Patient Navigator will provide high quality customer service to our patients and their families. Duties and Responsibilities:
  • Collaborate with physicians, and other clinical personnel in patient evaluations and treatment to further their understanding of significant social and emotional factors underlying patient's health problems
  • Conduct new patient's assessment screenings
  • Facilitate communications between the patient, family members and healthcare providers to
  • ensure patient satisfaction and quality of care
  • Inform patients of our NYPC services and provide them health education materials
  • Keep patients and physicians aware on insurance authorization status and be the main point of contact
  • Keep patients informed of progress of scheduled appointments
  • Provide comfort and support to patients
  • Document patient encounters in the EMR
  • Monitor patient's adherence to their medical appointments
  • Educate the patient on the policies concerning covered services
  • Actively utilize quantitative measures to monitor patient progress
  • Provide critical feedback to other members of the clinical NYPC team based on their observations
  • Schedule patient simulation appointments in a timely manner
  • Monitor Patient Satisfaction Surveys/Complaint
  • Comply with all New York Proton Center policies related to Privacy Rules established under the Health Information Privacy and Portability Act of 1996 (HIPAA)
  • Perform other job-related duties, as assigned
Qualifications:
  • High School graduate or equivalent
  • Minimum of 3 years medical office, or clinical environment experience, preferred
  • Ability to identify resources and utilize problem solving skills in order to meet patient's individual needs
  • Strong written and verbal communication skills
  • Attention to detail and problem-solving skills
  • Ability to work both independently and in a team environment.
