Job Description :
  • Serves as a linkage between patients and primary care providers to improve the coordination of care.
  • Collaborates with physicians, clinic staff and other interdisciplinary staff to identify care needs and connect patients with appropriate services.
  • Coordinates care for patients who have gaps in care, or quality flags, to close these gaps by outreaching, engaging and scheduling patients.
  • Coordinates patients scheduling, cancellations, and re-scheduling.
  • Communicates with patients through telephone, mail, MyChart and text messages where appropriate, to increase patient attendance.
  • Answers the telephone in a timely manner, politely and in a professional manner.
  • Meets with patients, primary care and BH providers and other staff as needed to coordinate care.
  • Encourages patient self-management with a focus on the achievement of care plan goals.
  • Documents and maintains patient-specific records.
  • Maintains accurate daily logs for statistical reporting purposes and for weekly reviews of productivity.
  • Meets leadership when necessary to discuss various concerns involving the functions and performance of the Ambulatory Care Department.
  • Completes mandatory hospital in-services via Healthstream.
  • Log in to E-Mail, and any other necessary program daily.
  • Performs any other duties as assigned.
Education Required - High school diploma, Bachelors preferredLicense/Certification/Registration(s) Required - NoneExperience/Skills Required:
  • Minimum of two years' experience in the healthcare industry
  • Advanced understanding of medical terminology and administration processes.
  • Proficient in information management programs and MS Office.
  • Outstanding communication and interpersonal abilities.
  • Strong attention to detail with excellent organizational skills.
  • Demonstrated proficiency in English and Spanish, preferred.
  • Ability to work effectively in a team.
