OverviewBlue Ridge Health is currently seeking a Patient Navigator to be part of our Clinical Team. This position is responsible for facilitating improved health care access and quality for BRH patients through advocacy and care coordination to address social determinants of health (SDH). Responsibilites include:* Screens patients for the eligibility of and assists with completion of needed applications and paperwork for: sliding fee scale/discount fee program subsidized health insurance programs charity care programs, including local hospital applications other programs that assist with social determinants of health (i.e. food insecurity, housing insecurity, transportation insecurity) medication assistance programs addressing legal issues* Conducts enrollments into programs when possible, and provides referrals to partner agencies, as appropriate.* Maintains certification as a Certified Application Counselor for the organization and completes all required training(s) to maintain certification.* Provides Senior's Health Insurance Information Program (SHIIP) counseling to Medicare beneficiaries, as requested.* Educates patients on health center advocacy and voter registration and provides related assistance.What We're Looking For:* Bachelor's degree in healthcare or social services related field, preferred. Equivalent combination of education and experience is acceptable.* Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.* Ability to be certified as a Certified Application Counselor (CAC) and Senior's Health Insurance Information Program (SHIIP) counselor within one month of hire.* Valid driver's license and reliable transportation, required.About Blue Ridge Health:At Blue Ridge Health our mission is to improve Health, inspire Hope, and advance Healing through access to Compassionate, Affordable, and Quality Care.
