We are seeking a compassionate and dedicated Patient Navigator to join our team in our women's Residential Treatment Program (Marty Mann). In this role, you will work with a skilled clinical team consisting of Licensed Alcohol & Drug Counselors, Nursing staff, a Clinical Pharmacist, to provide high-quality care to residents. In this role you would assist patients in non-clinical services, setting patients up for success after completing treatment. You would connect patients with community resources, assist in job searches, securing housing, and finding social programs that can continue to assist them in recovery.
Summary:As a Patient Navigator you will perform a variety of non-clinical duties for counselors. Making referrals on behalf of clients. Provide information and resources to clients for mental health professionals, work, food, and clothing resources, etc. Assist Clients with scheduling appointments with Medical professionals, NA/AA groups, tracking Narcan Distribution. Review and maintain records.CADT is looking for someone with excellent organizational skills and experience working within the community to provide resources to those going through our residential program.
Essential Duties and Responsibilities- Assist Counselor, Nurses, and the Program Director with non-clinical duties.
- Connect patients with community resources.
- Maintain and update lists of community resources.
- Help patients set up medical rides.
Required Experience:- Must have a minimum of High School Diploma
- 2 years of experience working in a human services setting or,
- Associate degree in Human Services, Social Work, or Psychology.
- Knowledge of community resources and connections to align clients with necessary resources.
- Must have or obtain CPR & First Aid certification before having direct client contact.
- Must pass background studies issued by the Minnesota Department of Human Services.
