BASIC FUNCTION: Maintain ongoing tracking and appropriate documentation on referrals to promote and ensure patient safety. RESPONSIBILITIES:
- Verify that information from referrals is received and linked within our referral system to effectively close the referral loop.
- Ensure referred provider receives complete and accurate demographic, insurance, and clinical information when referrals are sent to outside providers.
- Contact review organizations and insurance companies to ensure prior approval requirements are met.
- Resolves pre-certification, registration and case-related concerns prior to a patient's appointment.
- Provide specific medical information to financial services to maximize reimbursement.
- Review details and expectations about the referral with patients based on what referring physicians need.
- Track referrals to Grand River and ensure that physician practices receive follow up documentation in a timely manner.
- Assist patients with scheduling with specialist doctors when requested.
- Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.
- Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services.
- Acts as a liaison between hospitals, physicians, health plans, vendors, and patients, or other referral sources.
- Enters referrals and documents communications, actions, and other data in an information system. Maintain positive, professional, and helpful demeanor and always put patient needs first.
- Performs a variety of tasks and other duties as assigned.
QUALIFICATIONS: - High school diploma or equivalent preferred.
- 2 - 3 years experience in clinic setting preferred.
SALARY INFORMATION :$20.44-$28.74 Dependent on Experience
