We are seeking a compassionate and dedicated New York State Licensed Practical Nurse (LPN) to manage and oversee the medical care of adolescents in residential and family foster care. The LPN will play a critical role in promoting and restoring the health of youth by completing the nursing process, collaborating with physicians and multidisciplinary teams, and providing physical, emotional, and psychological support to both the youth and their families.
Key Responsibilities:
- Conduct health assessments through home visits and direct interactions to identify the needs of adolescents in care and their caretakers.
- Coordinate care by establishing medical homes and acting as a liaison between healthcare providers.
- Foster a supportive environment for youth and families, offering education on conditions, medications, and self-care skills to promote independence.
- Ensure compliance by documenting and auditing medical records while maintaining communication with the healthcare team.
- Administer and monitor medications, resolving health-related issues through team-based strategies.
- Utilize and model the Sanctuary Model tools and principles to create a trauma-sensitive culture focused on safety and growth.
