Pediatric Social Worker

Ambulatory Care Center (ACC)

Part-Time Temporary

Position Summary:

Social worker will provide comprehensive psychosocial assessment and intervention for patients and families coping with complex, ongoing medical needs, in support of optimal health outcomes. Assignment includes pediatric rheumatology and pediatric orthopedics. This part-time (21 hours/week), temporary position reports to the Senior Director, Department of Social Work Programs. Position is temporary through end October 2024.

Specific Functions:

  • Conduct initial psychosocial assessments with pediatric patients and their families to assess their understanding and the impact of their medical concerns and treatment, including barriers to adherence, both in clinics designated for coverage.
  • Confer with health care team regarding referrals for psychosocial assessments to triage patient concerns related to social work interventions, including support services, emotional and family related concerns, entitlement and insurance issues.
  • Participate in ACC case conferences, to share relevant interdisciplinary communication to assist with an integrative approach to care.
  • Develop social work treatment plan, intervene and follow-up as appropriate with patient, family and health team, as needed to ensure optimal outcomes.
  • Intervene to provide immediate assistance with high-risk, advocacy and resource-related concerns, such as child safety, school related concerns, home care, housing, insurance, and financial issues. Collaborate with both internal programs (such as Financial Assistance, Insurance Advisory, etc.) and community-based agencies (such as ACS).
  • Provide comprehensive documentation of client contacts and interventions in the medical chart and generate appropriate reports to track contacts and outcomes.
Qualifications:

Self-directed social worker with experience in health care, with pediatrics and chronic illness preferred. The successful candidate will be passionately committed to providing outstanding patient care, clinically skilled, and highly committed to professional development as part of being an engaged and active member of the Department. Excellent verbal, written and interdisciplinary collaborative skills essential. LMSW or LCSW licensure required; bilingual/Spanish preferred.

Other Requirements: #LI-Onsite #LI-TA1 Pay Range - Minimum: USD $59.59/Hr. Pay Range - Maximum: USD $67.84/Hr.
