Responsible to provide peer support and teach self-management skills for Genesis patients.

Essential Functions:
  • Develops linkages and refers patients for additional supportive services.
  • Conduct adherence assessments with patients to provide education and reduce barriers to adherence.
  • Possesses and uses good judgment, has the ability to work to maintain effective relationships with individuals and organizations.
  • Participates in ongoing training and regional collaboratives to enhance knowledge and skills.
  • Participates in quality improvement trainings and activities to improve patient care.
  • Provides follow-up on patient referrals.
  • Demonstrates cultural competency in working with diverse populations.
  • Facilitates enrollment in benefit and entitlement programs.
  • Ability to travel to Sun River Health locations as needed.
  • EDUCATION/EXPERIENCE:

    High School Diploma/GED required. Associate preferred.

    2 years history of advocacy experience.

    Hourly rate: $20.00-$22.00.

    Candidate must be a Certified Recovery Coach or receive certification within one year of being hired for the position or have courses supporting the information relating to counseling, substance misuse, mental health, and Evidence-Based Practices, Prevention techniques, as well as Intervention that relate to the population being served.
