What you will do:

• Provide physician-directed occupational therapy services by, initially and periodically evaluating patients needs, establishing a goal-directed plan of care, and initiating planned care. The plan of care and re-evaluation occurs no less often than every 60 days.
• Provides quality care within guidelines of department policy and procedures as well as local, state, and federal rules and regulations and the Code of Ethics as established by the American Occupational Therapy Association.
• Provides written documentation of assessment and care delivery in a professional manner according to policy and within the designated period.

Ideal candidate:

• Active NYS Occupational Therapy Licensure/Registration
• One year of occupational therapy experience is preferred, however, new graduates are welcome to apply
• Previous homecare experience and/or acute care or rehab experience is a plus
• OASIS Knowledge a plus
