Licensed Vocational Nurse - LPN / LVNJob DetailsJob TypeFull-time, Part-timeSan Antonio, TXDescriptionKlarus Home Health is looking for full time, part time or PRN / Per Diem Licensed Vocational Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the San Antonio, TX metro area.The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.RequirementsGraduate of an accredited School of Vocational/Practical Nursing.Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC.Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.EMR system: HCHB experience highly preferredProficiency in nursing skills as defined by generally accepted standards of practice.
