Position Summary:
We have an exciting opportunity to join our team as a Telephonic Care Nurse.
NYU Langone Department of Medicine Division of Geriatric Medicine and Palliative Care presents an exciting opportunity for nurses passionate about delivering telephonic palliative care management In this role the successful candidate will provide palliative care consultations to Patients Living with Alzheimers Disease and other related dementia after discharge from the Emergency Department or Emergency Department Observation Unit Nurse led telephonic case management will be conducted centrally for 82 participating clinical sites across the country. NYU Langone Health Key aspects of palliative care discussions will include establishing goals of care exploring end of life wishes discussing advance care planning with healthcare providers and addressing pain and symptom management The successful candidate will receive proper training on telephonic administered care to ensure appropriate patient clinician communication and effective palliative care delivery The nurse will work under the supervision of a board certified palliative care physician
Job Responsibilities:
* Quality/Performance Outcomes:
* Participates in center based quality and performance improvement program consistent with service and departmental programs
* Maintain awareness of current evidence based practices and uses research data applicable to patient care
* Participates in clinical inquiry and activities to foster an evidence based practice
* Participates in quality improvement programs and activities that promote patient family/caregiver physician and staff satisfaction
* Participate in 4 hour weekly learning network interdisciplinary team meeting
* Adhere to legal and ethical standards in palliative care ensuring patient confidentiality informed consent and compliance with regulations
* Clinical:
* Conduct thorough initial assessments during telephonic consultations considering the specific needs and conditions of Persons Living with Dementia and their care partners
* Develop individualized care plans based on the assessments prioritizing issues identified in collaboration with the PLWD and care partner in accordance with Aliviado Dementia Care training
* Implements interventions identified in the plan of care
* Collects patient healthcare data systematically Maintain accurate and detailed documentation of all telephonic consultations assessments care plans and interventions
* Perform medication reconciliations ensuring accurate and up to date medication lists and address any concerns or discrepancies
* Evaluate patient progress toward attainment of expected outcomes over 6 month periods
* Educates the patient and caregivers to facilitate and promote healthy adjustment to new or changed health patterns
* Considers needs and behaviors of specific patient age and cultural groups in all patient care activities
* Schedule and conduct follow up meetings after interdisciplinary team discussions reviewing care plans and making necessary adjustments
* Coordination/Leadership:
* Coordinates and self manage patient deliverables weekly
* Supports the missions philosophy standards goals and objectives of the institution and NYU Langone Division of Geriatric Medicine and Palliative Care
* Demonstrates knowledge of the NYUMC Service Standards and incorporates into the performance of responsibilities
* Participates in designated activities related to standards and regulations of relevant outside agencies
* Functions as a respectful member of the health care team
* Collaborate with interdisciplinary teams including physicians social workers and other healthcare professionals to ensure coordinated and holistic care
* Education/Development:
* Attends trainings on delivering telephonically administered palliative care
* Acquires and maintains current knowledge and competence to fulfill responsibilities
* Develop and enhance communication skills for effective engagement with PLWD care partners and interdisciplinary teams
* Focus on cultural competence to ensure sensitivity to diverse backgrounds
* Engages in self assessment and participates in activities to promote own professional growth and development
* Utilizes resources to meet individual learning needs and promote professional development in clinical practice
* Develop proficiency in telehealth technologies and electronic health record platform
Minimum Qualifications:
* To qualify you must have a A Baccalaureate degree in Nursing is highly preferred; a Diploma or Associate Degree in Nursing requires a minimum of one year experience in conjunction with plans to earn a baccalaureate degree required.
* Current Registered Professional Nurse in New York licensure is required multiple state licensures preferred
* Palliative or hospice medicine certification greatly preferred Willingness to obtain palliative or hospice medicine certification if does not currently possess
* Willingness to obtain licensure for multiple states if does not currently possess
* Fluent written and spoken English language skills necessary
* Experience with case management telephonically administered care or telemedicine preferred.
Preferred Licenses: Hospice/Palliative Nurse-Cert
Preferred Qualifications:
Palliative or hospice nursing certification licensure across multiple states experience administering telephonic care management
Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $43.39 - $55.00 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
