Job Summary: As a Personal Care Assistant for LIFE Geisinger, you will play a vital role in assisting participants in maintaining a healthy and independent lifestyle. You will provide personal care assistance both at the Adult Day Health Center and in participants' homes.
Key Responsibilities:
- Deliver individualized care according to personal care plans, allowing participants to maintain maximum independence.
- Provide direct support for personal care needs, including assistance with nutrition, hydration, vital signs monitoring, ambulation, transfers, medication compliance, and incontinence care.
- Engage in indirect care tasks such as light housekeeping, laundry assistance, and the supply delivery required for participant care.
- Be available for on-call coverage to handle after-hours emergencies as needed.
- Ensure the safety of participants and yourself through proper body mechanics and use of assistive devices during transfers and ambulation.
- Collect data for quality improvement initiatives and participate in action plans to enhance the LIFE Geisinger program.
- Maintain accurate documentation of services provided in compliance with program policies.
- Communicate effectively with older adults and collaborate with team members to deliver high-quality care.
- Explain care procedures to participants before performing tasks and offer choices to promote their independence.
- Attend interdisciplinary team meetings and complete all required training sessions.
Work Schedule: Full-time, Days; 7 AM to 3:30 PM, with travel to participants' homes and approximately one weekend shift every 7 to 8 weeks.

Education & Experience: High School Diploma or GED required. Minimum of 1 year of related experience preferred. Certifications in CPR and Basic Life Support, along with a valid driver's license are necessary.
