Primary Function:The Personal Care Attendant PC is a caring and dependable individual who assist elderly and/or disabled persons in their homes or places of residence. Personal care Attendant Backup works on an as needed basis as backup to assist with patient needs. This includes non-medical personal assistance, skilled nursing and rehabilitation and hospice and palliative care. This role works on a very limited basis and/or as needed basis.Job Responsibilities:General Duties and Responsibilities:Adheres to Organization's Policy and Procedures.Acts as a role model within and outside the Agency.Performs duties as workload necessitates.Maintains a positive and respectful attitude.Communicates regularly with supervisor about Department issues and keeps management advised of potential problems in all areas.Demonstrates flexible and efficient time management and ability to prioritize workload.Consistently reports to work on time prepared to perform duties of position.Meets department productivity standards.Participates in working groups, councils and committees.Ensures compliance with rules and regulations.Participates on committees, and special projects, and seeks additional responsibilities.Accomplishes all tasks as appropriate.Recommends new approaches to effect on-going continual improvements to policies, procedures, and documentation.Maintains compliance with federal and state regulations concerning employment.Participates in administrative staff meetings and attends other meetings and seminars.Create and conduct presentations using audiovisual tools including power point.Performs other related duties as required and assigned.Consistently promotes the company's core values.Completes required compliance annual training.Essential Functions of Position:Focus on the patient to deliver the appropriate care and emotional support to the patient and their families.Perform the day-to-day activities of care giving in accordance with current regulations governing Home Health Care.Follow care guidelines outlines in the patient's individualized care plan. This can include personal care assistance, Alzheimer's/Dementia Care, stroke recovery, ambulation/mobility assistance, transportation needs, meal preparation, and housekeeping.Documents care provided and complete the forms required for the patient's records.Report to the supervisor any observed or reported changes in the patient's condition and/or needs in a timely manner.Maintains and keep confidential private client information and propertyCommunicates effectively with all members of the team.Report client incidents/accidents to a supervisor immediately (same day).Report and respond to emergencies in a calm manner, I,E., calling 911 and notifying a supervisor.Report to a supervisor any suspected abuse, neglect or exploitation.Report to a supervisor for any reason the client is not found to be at home, not responding to the door, or if the client leaves on vacation.Report to a supervisor any unsafe activity or hazardous situation found at a client's home or property.Education:Licenses/Certifications:CRNA certificationCPR certification desiredExperience:Have at least six (6) months' paid work experience as an agency homemaker, nurse aide, maid or household workerAt least one (1) year experience, paid or unpaid, in caring for children or for sick or aged individualsSuccessful completion of formal training in nursing arts or as a nurse aide or home health aideThis role requires full COVID-19 vaccinationPhysical Demands:Fast-paced cubical office environmentSome stress may occurWhile performing the duties of this job, the employee is regularly required to sit for extended periods of ime, and to speak and hear. The employee is required to stand and walk. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.
