This position's primary duties will be coordination of services for clients on who seek services on the mobile units, including transporting to appointments, assistance with paperwork completion, working closely with two Community Service Boards to reduce barriers, provide education and training to clients, serve as a pseudo case manager to ensure clients are enrolled in appropriate services and can access social support resources. The selected candidate will need to work non-traditional hours, including weekends and be onsite at all events across all 8 of our localities, Bristol City, Washington County, Smyth County, Wythe County, Bland County, Carroll County, Galax City and Grayson County.The Virginia Department of Health offers 12 paid holidays, medical, dental, vision and life insurance, retirement plans (including 401a Cash Match and 457 Deferred Compensation Plan) as well as sick, family/personal, and annual leave. Facilitates service delivery to clients in a health district or other direct service work unitby interpreting language to staff and clients. Duties typically associated with overallservice delivery are well defined include securing patient history, providing basic healtheducation, explaining various routine assessments, diagnostic and treatment processesas well as identifying other social or financial service providers to contact for assistance.May provide transportation for clients or provide general administrative tasks such askeyboarding, filing, reception.Minimum Qualifications•Ability to prepare required pre-clinic paperwork to maintain clinic flow, assistwith labeling and packaging lab specimens•Good interpersonal relations skills including cultural sensitivity•Office experience with public contact, keyboarding, filing & organizing ownworkload, working knowledge of computer applications•Experience in health fairs and other community activities•Ability to communicate with other agencies, assist clients to overcome barriersto treatment and maintain accurate records and reportsAdditional Considerations•Experience in health care settings and/or occupationsSpecial InstructionsYou will be provided a confirmation of receipt when your application and/or rsum is submitted successfully.
