Job Description :

We are looking for a Physical Therapist to work for one of our external clients, a community services non-profit organization serving people with developmental disabilities for their clinic, located in the Bronx.

In this role you will be responsible for planning and implementing prescribed physical therapy treatment programs for people receiving services to maintain and/or restore function secondary to acute or chronic disability; conducting annual screens, assessments, and daily documentation if warranted.

Requirements:

  • Graduate of an accredited occupational therapy assistant school.
  • New York State license, limited permit, or eligible for licensure to practice physical therapy.
  • Knowledge in PT evaluation and treatment; ability to supervise direct assistants and aides; understand the principles of physical agent modalities employed in PT; knowledge in the use of mobility devices and therapeutic exercises.

Responsibilities include, but not limited to:

  • Reviews MD's referral and persons receiving services medical record prior to determining PT treatment required. Refers person to specialty clinics as needed (adaptive equipment, shoe and splint). Maintain active/up to date MD prescriptions as outlined by NYS Department of Health (DOH) and Office for People with Developmental Disabilities (OPWDD) depending upon work assignment.
  • Evaluates and assesses the persons' needs and level of functioning in the areas of: sensorimotor, gross & fine motor skills, musculoskeletal status, CNS status, ambulation and adaptive equipment needs (e.g. walker, canes, AFO's). Evaluates persons receiving services within facility-specified time frame.
  • Confers with physicians and other health care practitioners to obtain additional information. Integrates input from persons receiving services, family, and direct care staff in treatment assessment and recommendations.
  • Devises treatment plans based on the assessment; administers treatment involving the application of modalities: hydrotherapy, hot packs, cold packs, ultrasound, and electrical stimulation; instructs, motivates, and assists persons receiving services in various exercise programs utilizing weights, exercise machines, mat activities, assistive devices, orthoses, and prostheses.
  • Writes specific, objective, and measurable documentation, which are outcome oriented and details the person's needs, potential to benefit from treatment and progress achieved to date. All documentation must be completed through ECW within the time frame established by the Healthcare Center. Record treatment, response, progress and outcome/supportive programs.
  • Consults with supervisor when appropriate, to implement recommendations.
  • Provides training to persons receiving services, families and/or direct care staff in specified follow through programs.
  • Communicates effectively with staff regarding discharge planning from a rehabilitative program.
  • Participates and attends care plan/team meetings as needed for persons receiving PT services.
  • Supervises PTAs & therapists on limited permits, and performs oversight in the Healthcare Center, day programs and residential settings as applicable.
  • Performs all other duties as assigned.
