Job Description :

The Physical Therapist is responsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients. Responsible to supervise Physical Therapy Assistants and cosign their documentation when required. Supports organizations mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.

JOB DUTIES / RESPONSIBILITIES

ESSENTIAL DUTIES

1. Provides skilled physical therapy in accordance with the authorized practitioner's plan of care.

2. Treats the patient through the use of therapeutic activities designed to remediate musculoskeletal deficits and restore function . Demonstrates and teaches proper transfers, ambulation, positioning and all other PT functions as stated in their practice act.

3. Accepts only patients referred by the Team Supervisor, Case Placement Coordinator or designee.

4. Demonstrates competency in skills required by the agency on at least an annual basis at the time of field evaluation.

5. Evaluates and assesses the patient's level of function by applying diagnostic and prognostic procedures and tests and assists in the development and revision of the plan of care. Assesses the patient's continual care needs. Ensures that the plan of care addresses all problems identified in the assessment or documents rationale for not doing so.

6. Identifies specific DME/orthotic needs of patient and refers to appropriate vendor.

7. Observes, records and reports the patient's reaction to treatment and any changes in the patient's condition to the RN or RN Coordinator or Team Supervisor. Reports routinely every 30 days to RN, RN Coordinator or Team Supervisor. Requests referrals for other disciplines as indicated by the needs of the patient or documents rationale for not doing so.

8. Teaches, supervises and counsels the patient, family and other health care team members regarding home rehabilitation procedures, including the home exercise program, and other care needs as appropriate to the patient's condition.

9. Evaluates home safety and coordinates with team members to address issues identified.

10. Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients.

11. Attends patient care conferences. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed to ensure good coordination of care.

12. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies.

13. Submits all visit notes every week or sooner. Submits 60day, D/C summaries and/or quarterly reports as required by the individual program and department.

14. Co-signs all medical record documentation for the Physical Therapy Assistant (PTA) in the EMR in a timely manner.

15. Provides clinical supervision to PTA as per regulation.

16. Provides educational clinical support to the PTA to ensure safe effective treatment of patients.

17. Utilizes agency educational material as appropriate.

18. Meets weekly productivity standards and demonstrates flexibility in scheduling patients as required by department.

NONESSENTIAL DUTIES

19. Cooperates with appropriate staff to provide staff education when requested and arranged by the Team Supervisor.

20. Attends meetings and serves on a variety of patient care and strategic planning committees as assigned by the Team Supervisor or Associate Directors.

21. Participates in the agency's quality improvement program.

22. Attends all staff meetings as required.

Successful completion of a physical therapy program at a school of physical therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or both. If licensed or registered prior to 1966, meets the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12 (1) (2) (3) .

Licensed in NYS as a Physical Therapist.

Previous experience required:

Minimum of one year of experience in pediatric physical therapy and/or pediatric affiliation.
