Overview:

Practice and promote the agencys mission statement in every interaction. Provide ongoing support to people in becoming active participants in every aspect of their lives and to ensure that a high quality of life is maintained. Performs initial Physical Therapy evaluations and completes evaluations and all follow-up evaluations. Provides direct PT intervention as needed, develops appropriate PT programs. Completes temporary service plans, 21-day reviews, and quarterly and annual reviews. Provides staff training as needed. Responsibilities:

Primary duties and responsibilities include but not limited to:

  • Practice and promote Citizens mission statement in every interaction. Through family and community engagement, Citizens supports people to live the life they choose.
  • Read through email and respond to email within a 24 hour period.
  • Perform as needed reassessments due to the continual changes in status of the individuals that reside at Helen Kaplan ICF.
  • Perform annual reassessments and update PT evaluations.
  • Write the Comprehensive Functional Assessment yearly for all individuals and when individuals change to Medically Frail house.
  • Update the PT guidelines, staff actions and informal programs for each service recipient as part of the yearly assessment.
  • Revise PT evaluations whenever a PT program requires modification.
  • Revise the guidelines, staff actions and informal programs as needed.
  • Write quarterlies for all individuals receiving any PT directives.
  • Discuss treatment sessions with PT Assistant and approve treatment activities.
  • Weekly observe provision of direct PT Assistant intervention.
  • Assist with ordering all wheelchairs and adapted equipment, including completing all wheelchair medical justifications.
  • Assist with contacting seating specialists and vendors for equipment repairs.
  • Complete online research for equipment and medical information.
  • Review prescriptions for direct treatment and direct treatment plans.
  • Review and co-sign daily notes maintained by PT Assistant for all direct treatment sessions.
  • Meet with IDT members (QIDP, OT, SP, BIS, NUR) as needed to ensure interdisciplinary care for individuals.
  • Speak with parents, siblings and guardians about PT intervention, equipment, braces, etc. when requested.
  • Assist PT Assistant with maintaining equipment lists for all equipment including wheelchairs and seating systems, mechanical lifts and bathing surfaces.
  • Review paperwork from hospital, nursing home and or prior residence for new admits. Contact PT staff from previous setting as needed for prospective individuals.
  • Write the Temporary Service Plan for prospective individuals.
  • Complete PT assessment upon admission.
  • Develop PT programs (guidelines, staff actions and informal goals which include but are not limited to wheelchair use and proper positioning, ambulation safety, transfer safety, bathroom safety, dining safety with regard to seating, seating safety, etc.)
  • Write 21- Day Reviews for all new service recipients.
  • Perform review of all PT paperwork for annual audit.
  • Report to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect. Additionally, must follow all agency incident policy and procedures.
  • Adhere to Federal and New York State regulations, agency policy and procedures, the employee handbook and agency and program training requirements.
  • Successfully complete and maintain annual training and other training that is necessary for the Physical Therapist position.
Qualifications:
  • Must be a current New York State Registered Physical Therapist.
  • Minimum of 3 years related work experience in the field with Developmentally Disabled Adults
  • Minimum of 3 years experience in a supervisory role.
  • Must be proficient in computer skills
