Job Summary: We are seeking a skilled Physical Therapist (PT) to join our team in providing exceptional rehabilitation services to residents. As a PT, you will assess, plan, and implement personalized therapy programs to enhance mobility, strength, and overall quality of life for patients in our skilled nursing and rehab facility. Essential Duties:
- Conduct assessments to determine physical therapy needs and goals.
- Develop and implement individualized treatment plans for residents.
- Provide hands-on therapy to improve strength, balance, and mobility.
- Collaborate with the healthcare team to optimize patient care.
- Document progress and modify treatment plans as needed.
- Educate residents and their families on techniques for improving daily function.
- Adhere to infection control practices and regulatory standards.
Required Skills:
- Current Physical Therapist license in New York State (NYS).
- Strong assessment and therapeutic skills.
- Ability to work effectively in a multidisciplinary team.
