About the Role: As a Physical Therapist, you'll play a critical role in helping patients restore mobility, reduce pain, and improve overall physical function. This position provides the flexibility to work on an as-needed basis, delivering care to patients of diverse ages and backgrounds. Primary Responsibilities:
- Develop and implement individualized physical therapy plans for patients based on medical prescriptions and assessments.
- Guide patients through exercises and therapeutic techniques to achieve treatment goals.
- Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care.
- Monitor and document patient progress to adjust treatment plans effectively.

Qualifications:
- Graduate of an APTA-approved school of physical therapy.
- Valid Physical Therapist license in the state of Illinois.
- CPR certification required (available upon hire).
- Valid driver's license required for travel between facilities as needed.
- Strong interpersonal and communication skills, with a patient-centered approach to care.
