We are seeking a compassionate and dedicated Occupational Therapist (OT) to join our home care team. In this role, you will be responsible for providing personalized therapy services to patients in their homes, helping them regain or maintain their independence in daily activities. As a Home Care OT, you will be driving to different locations to deliver high-quality care, focusing on both assessments and routine visits.

Key Responsibilities:

  • Patient Assessments: Conduct thorough initial assessments to understand each patient's functional abilities, challenges, and goals. Develop personalized care plans tailored to their specific needs.
  • Therapeutic Interventions: Implement and oversee therapy plans designed to improve patients' ability to perform daily activities, such as dressing, bathing, cooking, and other tasks essential for independent living.
  • Routine Visits: Provide ongoing support and therapy during routine visits to ensure patients are progressing according to their care plan. Adjust interventions as needed to maximize patient outcomes.
  • Patient and Family Education: Educate patients and their families on techniques to improve daily functioning, the use of adaptive equipment, and strategies to manage their condition at home.
  • Collaboration: Work closely with other healthcare professionals, including physical therapists, nurses, and physicians, to coordinate comprehensive care for patients.
  • Documentation: Maintain accurate and timely records of patient assessments, treatment plans, progress notes, and any other relevant documentation in compliance with regulatory standards.

Qualifications:

  • Licensed Phsyical Therapist (PT) in [NY].
  • Minimum of [1] years of experience in occupational therapy, preferably in a home care setting.
  • Strong understanding of home care practices and the ability to work independently.
  • Excellent communication and interpersonal skills.
