Physical TherapistLocation: On-site, Philadelphia, Pennsylvania, United StatesSalary: $40 - $55 per hourJob Description:Join our client in providing high-quality outpatient rehabilitation services directly in patients' homes or independent living facilities. As a Physical Therapist, you'll play a key role in evaluating, treating, and enhancing patient performance in a familiar environment. This position offers a flexible schedule and a supportive work environment where your skills and experience can thrive.Key Responsibilities:

• Provide comprehensive physical therapy services in home settings.
• Evaluate and develop personalized therapy plans for patients.
• Supervise Physical Therapy Assistants and other support staff as necessary.
• Attend staffing conferences and contribute to patient care decisions.
• Collaborate with other rehabilitation team members to create optimal therapeutic plans.
• Promote patient safety and eliminate hazards.
• Provide education on chronic disease management and falls prevention.
• Perform routine reexaminations to update treatment plans.
• Maintain compliance with HIPAA, federal regulations, and organizational standards.

Qualifications:

• Valid Physical Therapy license in Pennsylvania or proof of license eligibility.
• Master's degree in Physical Therapy or a related field (Bachelor's degree plus experience is also considered).
• Valid driver's license or reliable transportation for patient visits.
• Strong organizational, time management, and communication skills.
