Physical Therapist (PT)Clewiston Nursing and Rehab is a 155 bed skilled nursing facility with a supportive family environment. Our compassionate rehab teams are led by an experienced Director of Rehabilitation. Rehabilitation techs are utilized to assist therapists with resident transport and administrative functions. The rehab department utilizes Point Click Care and Optima Therapy for billing and documentation to assist therapists with daily efficiency where quality care remains the primary focus of each therapist. The facility has a 7-day a week, in-house comprehensive rehabilitation program that is an integral member of the interdisciplinary team. Our rehab positions are stable and secure at one location. Each rehab department offers spacious gym space. EXPERIENCE MAKING A DIFFERENCE, ONE PATIENT AT A TIME MAKE THE DECISION TODAY, FOR A BETTER TOMORROW: One Location, No Daily Travel Flexible Schedules (Per Diem) Direct Deposit Summary of the Physical Therapists (PT) The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Essential Duties & Responsibilities of the Physical Therapists (PT):
  • Creates, implements, and executes patient treatment plans.
  • Communicates with patients, families, referring physicians, and other members of the team.
  • Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
  • Develops and enhances clinical and professional skills through knowledge and professional associations.
  • Supports and participates in department operations and development.
  • Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
  • Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
Job Requirements of Physical Therapists (PT): • Current Florida License as a PT (Physical Therapist) • Current CPR
  • At least one (1) experience working in a Skilled Nursing Facility preferred.
