Physical Therapist (PT)Compensation: $86,000 - $98,000 salary commensurate with experienceShift Type: Full-Time, Monday - Friday, 8:00 am - 4:00 pm (Flexible)Benefits:100% Paid Monthly Premiums for Medical, Dental & Vision InsuranceCell Phone Reimbursement3% Matched Simple IRAPTOPaid HolidaysiPad and supplies for patient visitsCompany OutingsBonus OpportunitiesPaid MileageSummaryThe Physical Therapist (“PT”) is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment. A PT provides therapy services, including the creation and administration of care plans, as well as oversees physical therapy assistants. A PT must work collaboratively across the internal interdisciplinary care team, physicians, and other stakeholders in our patient's care.Primary Responsibilities:Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.Provide and complete all proper documentation.Administer physical therapy treatments as ordered by a physician. Direct and oversee treatment given by physical therapy assistants.Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients' progress, and recommending adjustments and modifications.Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and / or providing equipment; recommending outpatient follow-up programs).Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.QualificationsMinimum of 1 year of physical therapy experience with a preference for 1+ years in an acute or a post-acute care setting.Active state Physical Therapist licensureValid CPR certificationValid Driver's LicenseAt Apollo Medical, we strive to provide a fulfilling career where each employee has rewarding opportunities to positively impact our patients' lives.
