Physical Therapist

Location: Sonora, Jamestown, & Cold Springs, CA area

Employment Type: Direct-Hire, Permanent

Setting: Home Care and Hospice

Salary: $130,000-$140,000 Schedule: Mon-Fri, create your own schedule (No holidays, weekends, or on-call)

Qualifications:

• Required: Doctorate or Master's Degree in PT, CA PT license, 2 years of experience, CPR, Driver's License and reliable vehicle with insurance

• Preferred: Community or Hospice experience, open to other backgrounds

Key Responsibilities:

• Provide comprehensive physical therapy services based on personalized care plans, focusing on seniors, post-surgical patients, stroke survivors, and individuals with cardiac conditions

• Assess therapeutic needs and evaluate patients' physical capabilities, including muscle strength and mobility

• Direct appropriate treatments to enhance patient comfort and functionality, and educate patients and families on therapeutic devices

• Report changes in patient conditions to physicians, engage in interdisciplinary collaboration, and participate in discharge planning

• Supervise PTAs/Hospice Aides and contribute to quality improvement initiatives
