- Qualifications:*
- Associate's Degree (Required)
- Physical Therapy Assistant License in MN (Required)
- CPR Certification (Preferred)*Primary Responsibilities*:
- Provide physical therapy to home care patients.
- Responsible for timely and accurate communication and documentation relative to physical therapy services.
- Promote physical therapy services in homecare*.**Specific Job Duties - includes, but not limited to:*
- Treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
- Execute an appropriate care plan for restorative and maintenance patients.
- Collaborate with physicians, case managers, nurses, occupational therapists, physical therapists, home health aides, and office staff to ensure consistent levels of care.
- Monitor patient progress and document accordingly.
- Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.*About Us:*Hiawatha HomeCare is a family-owned agency in Red Wing MN.
