However, it does not in any way state or imply that these are the only duties you will be required to perform.Provides Physical Therapy plan of treatments and modalities according to the established treatment plans as assigned, under the on-site supervision of a licensed Physical TherapistMust possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general publicMust effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairmentsMust possess the ability to train/supervise and discipline subordinate staffMust be able to relate information regarding a resident's conditionMust possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the departmentDocuments resident progress toward Physical Therapy goalsCompletes Daily Activity Schedule and progress notes per facility policy and as directed by the Physical TherapistProvides resident, staff and family education as directedComplies with Federal and State regulations related to rehabilitation therapyCompletes all documentation to comply with professional standard and regulatory requirementsAssists in ordering necessary equipment for the facility and residentsComplies with all facility policies and proceduresMaintains confidentiality of facility and resident informationReports all instances of potential resident abuse, neglect, and misappropriation of resident property immediatelyFollows sanitation and infection control policies and proceduresKeeps Facility Rehabilitation manager and Physical Therapist informed and updated appropriatelyACKNOWLEDGEMENTPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
