Job Title: Physical Therapy Assistant - in Teaneck, NJ Shift: Monday-Friday, or a modified shift Sunday - Thursday, Tuesday - Saturday. (Flexible, days) Job Description: The purpose of this position is to provide support to the Physical Therapist and Rehab Department in conducting therapy treatments to improve patients' mobility and physical health. Responsibilities: Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist. Communicate with the Physical Therapist, the patient, and/or family and facility staff any pertinent information concerning patient progress, problems, and plans. Participate in Patient Plan of Care Meetings and Interdisciplinary Team Meetings if asked by the Physical Therapist. Participate in the in-service training program for other staff in the facility. Record treatment notes, and progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure. Participate in discharge planning under the supervision of the Physical Therapist. Recommend any needed equipment to the Physical Therapist which would facilitate improved delivery of clinical services. Periodically work beyond usual working hours as needed for facility emergencies, weekends, and holidays.
