Placement Specialist - Youth Villages : Job Details

Responsible for all duties related to the coordination of internal and external placements for all NC programs. First point of contact for all referrals from external customers. Primary screener to determine eligibility criteria for YV models (Intercept, MST, Life. Set, Wraparound)Responsible for monitoring and tracking all referrals through the admission process. On-call to families while they are in the pre-admission phase. Primary liaison between YV programs, internal/external customers, referral agents, and funding sources. Provide ultimate customer service with all referral sources throughout the pre-admission process and maintain appropriate documentation of communication. Complete initial PCPs and/or serve families in a counselor role when designated. Maintain appropriate model training. Other duties as assigned Qualifications Requirements.
