Overview:
The One Brooklyn Health (OBH) system was established to preserve and enhance health care services in the communities of Central Brooklyn through the merger of three hospitals and their affiliated facilities- Brookdale Hospital Medical Center (BHMC), Interfaith Medical Center (IMC), Kingsbrook Jewish Medical Center (KJMC). All three hospitals have historic ties in the communities they serve and are now working together to build a system that will enable OBH to provide the highest quality of healthcare services and improve health overall for Central Brooklyn. OBH encompasses a total of 2800 beds over 14 locations including an extensive network of primary, behavioral health and specialty care locations spread throughout Central Brooklyn. At various locations, patients and their families can obtain a broad range of healthcare services, such as pediatric care, behavioral health, sickle cell services, podiatry, and maternal health services. In addition, OBH has two large nursing homes within its system, Schulman and Schachne Institute for Nursing and Rehabilitation at Brookdale and Rutland Nursing Home at Kingsbrook Jewish Medical Center.
Reporting to the SVP for Population and Community Health, this position works collaboratively across teams to plans, manage, and coordinate project activities to ensure goals and objectives of activities related to population health, community health and health equity are accomplished within prescribed timeframe and funding parameters.
As part of the ongoing work of One Brooklyn Health to transform healthcare delivery in Central Brooklyn, the Population and Community Health team works to eliminate racial and gender health inequities rooted in historical and contemporary injustices, by improving health outcomes, addressing social drivers and using data for advocacy.

Experienced with presenting technical data and information to non-technical users.
Knowledge and Skills:
Advanced skills in Excel and ease with data management concepts (i.e. MS Access, SQL).
