Reports To: Nurse Manager Department: Scofield Manor FLSA Status: Exempt (Full-Time) Shift: 10:00 AM to 6:30 PM (Monday – Friday) Date Adopted: January 1, 2017 Revision Dates: September 27, 2024, October 18, 2017 Position Summary: The Licensed Practical Nurse (LPN) provides medical care to residents at Scofield Manor. This position conducts assessments, plans and administers medication, and supervising care. Essential Duties and Responsibilities: Patient Care Management Oversees all direct medical care at Scofield Manor residents under the guidance of the Nurse Manager (RN). Communicates with residents about health issues including physical and mental health, as well as conducts monthly assessments of blood pressure and weight of residents. Transcribes doctor orders and implements any orders as necessary. Provides all LPN functions, such as, wound care, nebulizer treatments, diabetic testing, flu vaccines, etc. Coordinates and ensures completion of all medical appointments (eye, dental, mental health, etc.), tests and lab work as required. Schedules medical appointments and coordinates transportation. Assesses residents returning from a hospitalization. Assists in discharges planning and coordinates referrals for home care, physical therapy, or other needed services. Maintains and updates Care Plans for each resident. Responds to emergencies in a calm manner, assesses care needed, and acts accordingly. Medication Administration Orders resident medications; ensures accuracy of records. Ensures medication inventories ae done daily. Evaluates medication administration by direct care staff, to ensure accuracy and safety. Maintains and updates medication administration records to comply with state regulations. Training and Development Assists the nurse manager in identifying, proposing and developing in-service training programs for staff; documents training sessions. Documentation and Communication Maintains detailed, thorough records on each resident in accordance with state regulations. Collaborates with residents, families, staff, physicians and other healthcare providers to ensure effective communication and the highest quality of care. Supervisory Responsibilities Provides indirect supervision of Attendants. Other Adheres to all Local, State, Federal and Charter Oak Communities' policies, procedures, standards and codes. Supports scheduling of employees when there are uncovered shifts, when necessary. May be required to perform other related duties as required and/or assigned. May be required to work different shifts, per direction from the Nurse Manager, based on business needs. Knowledge, Skills and Abilities: Strong knowledge of gerontology, medical and mental health nursing. Excellent communication skills, both verbal and written with residents, families, staff members or members of the community in a respectful, patient manner. Ability to build and maintain positive relationships with healthcare providers and community members. Strong interpersonal, judgment, reasoning, time management and organizational skills. Extensive ability to communicate successfully with people from a broad range of backgrounds. Ability to work in a fast-paced, changing environment. Flexibility to work extended hours as needed. Minimum Education/Experience Requirements: State of Connecticut Licensed Practical Nurse (LPN) certification is required. Minimum of four (4) years LPN experience, preferably two (2) of which are in a managerial role. Experience working with diverse populations and providing a range of services. CPR certification is mandatory. Additional Requirements: Proficiency in Microsoft Word, Excel, and Outlook. Ability to pass an exam assessing competency levels as per position criteria. Must pass a pre-employment physical, drug screening, educational and criminal background check. COVID-19 vaccination and boosters are mandatory.
