Essential Job Functions:

* Accepts referrals in timely manner, completes screening, intakes and follows up on no shows.

* Establishes and maintains inter-agency relationships to promote services in the community.

* Links identified youth and families to community-based services and completes required prevention/intervention documentation within ten days.

* Develops and Implements referral tracking system to ensure services are being delivered in a timely manner.

* Develops and implements evidence based prevention groups such as anger management, social skills, coping skills, etc.

* Participates as community liaison, and in prevention activities and school based meetings to assist with appropriate service placement.

* Works collaboratively with other community agencies to develop, plan and assist with implementation of evidence-based substance abuse education programs.

* Provides general case management services for youth on an as-needed basis.

* Provides intervention services in time frame established by State.

* Provides prevention services when needed.

* Advocate for clients' rights and preferences.

* Provides supportive counseling services to child/adolescents who are identified as at risk for substance abuse issues.

* Plans and facilitates prevention based groups to individuals who are identified as at risk for substance abuse issues.

* Documents all activities as required by agency policies and procedures; submit legibly written progress notes and logs to medical records within twenty-four hours of service.

* Complies with all state and federal requirements for confidentiality of substance abuse records.

* Completes billed hours reporting in data systems, and meets productivity as assigned by Supervisor.

* Develop and provide staff related training on risk factors.

* Additional duties may be required as per specific program guidelines.

Minimum Education and/or Experience: Graduation from an accredited school with a Bachelors' Degree in Psychology, Social Work, or other relevant Human Services discipline. Certified Prevention Professional (CPP) preferred. Master's Degree preferred. Additional education and/or experience may be required as per specific program guidelines.

Knowledge/Skills/and Abilities:

* Skill in time and organizational management; ability to complete work under strict time lines, while maintaining flexibility to assist with client issues/concerns.

* Ability to quickly assess situations and use sound judgment in diffusing potentially risky situations; ability to identify services needed by clients during the rehabilitation process.

* Ability to follow both oral and written instructions and complete tasks as instructed.

* Ability to express oneself clearly and concisely both orally and in writing.

* Ability to prepare clear and concise written reports.

* Ability to communicate clearly with clients to develop a supportive and trusting relationship.

* Ability to establish and maintain effective working relationships with co-workers, medical and clinical professionals, educational systems, court systems, and the general public.

* Knowledge of community resources.

* Knowledge concerning current information in the area of mental illness and substance abuse and treatment and related behavioral problems specific to the age and population served.

Necessary Special Requirements: Possession of a valid Florida driver's license, acceptable driving record, and proof of personal automobile insurance if required to drive an SMA vehicle and/or use a personal vehicle for SMA business. Complete State of Florida mandatory background screening prior to start of employment. Complete SMA required training during the first six (6) months of employment and updated if required.

Physical: Mobility and ability to bend and reach during an 8-12 hour day. Able to lift minimum 10 pounds. Visual and auditory acuity sufficient to evaluate, intervene, treat, and record client health care needs. Fine motor skills for legible and accurate charting, daily correspondence and presentation, either manually or orally. Work endurance ability to work 8-12 hour shifts with a meal break, as possible. Routine 8-12 hour shifts. Hours and days off may vary. Extra hours may be required. Work assignment locations may vary.
