JOB DEFINITION:The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at CN Guidance and Counseling Services, which serves adolescents, and young adults who are experiencing first episode psychosis. Our mission is to empower young people to make meaning of their experiences and to pursue their goals for school, work and personal relationships. Equity, inclusion, rapid access and self-determination are at the core of everything we do.The Primary Clinician works closely with all members of the team in an integrated way, develops a positive clinical relationship with the client early on, and facilitates linkage with other community supports.EDUCATION REQUIREMENT:Licensed Clinical/Masters Social Worker, Licensed Marriage and Family Therapist, or Licensed Mental Health CounselorEXPERIENCE REQUIRED:1-year experience in mental health and/or chemical dependency community-based settingDUTIES AND RESPONSIBILITIES:
  • Primary resource for participants and their families to provide emotional and practical support focusing on participant outreach, engagement, and retention.
  • Conducts needs assessments; provides psycho-education regarding coaching sessions that focus on skill building; engages in safety planning and wellness management planning and connects participants and families with the support and services they need.
  • Conduct clinical assessments to determine the participant's level of social functioning and substance use via a shared decision making (SDM) framework.
  • Draws on expertise of other team members and deploy` s team resources
  • Gather sufficient information for each individual to make a diagnostic assessment necessary to determine assignment priorities and treatment plan.
  • Engage in safety planning and wellness management planning with
  • Connect participants and families with the services and supports they need;
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
  • Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
  • Provide supportive therapy
  • Develop and modify treatment using a shared decision-making process with clients
  • Work with participants and family for transition from the team to other services and supports, and ensure stable links at discharge.
  • Rotating on-call responsibilities with other team
  • Engage in performing assessments, handling crisis intervention and discharge
  • Provide direct, face-to-face and/or collateral services to individuals either in the OnTrackNy office or in the community in places and situations where problems
  • Regularly assesses with Team Leader the progress of individuals.
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.
  • Identify and implement quality improvement trainings and activities as part of professional development goals.
  • Encourage quality improvement processes and provide consistent resources for QI and professional development.
  • Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
