JOB SUMMARYUnder the direction of the Behavior Support Coordinator this position is responsible for assisting with the implementation of Outreach/Advocacy activities per the FSS Outreach and Advocacy Contract. This position will also plan and provide services within the ICS (Individual Community Supports), as well as other services as necessary. This position provides services to people supported through the OMH Advocacy contract. JOB FUNCTIONS:Regulatory and Corporate Compliance
  • Ensure compliance with regulations, agency policies, procedures and OPWDD/OMH regulations.
  • Complete all required documentation accurately, legibly and in a timely per regulations.
  • Follow the agency's incident management process
  • Ensure the Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures.
  • Maintain a working knowledge of OPWDD and OMH regulations applicable to assigned contracts.
Financial Management
  • Accurately document services provided under the Outreach and Advocacy FSS and OMH Advocacy Contracts to fulfill contract requirements and ensure payment.
  • Complete reports required by Agency and funders/regulators to support billing and service provision per contract regulations.
  • Meet unit and revenue targets for contracts while adhering to budgetary guidelines.
  • Maintain thorough and timely billing documentation in alignment with agency and regulatory standards.
Person Centered Services
  • Provide services as outlined in individuals' Life Plan (LP), including Individual Protective Oversight Plan (IPOP), Residential Staff Action Plan, Support Plans, Day Habilitation Plans, Community Habilitation Plans, Staff Action Plans, ICS and Respite plans and other safeguards and ensure all related documentation is completed.
  • Implement activities as outlined in the Staff Action Plan and, as applicable, the Life Plan.
  • Plan and coordinate daily activities to meet the needs and preferences of the people supported.
  • Act as an advocate for people receiving support from the agency, respecting their rights to privacy and confidentiality while using appropriate written and verbal communication.
  • Provide person-directed services.
  • Complete assessments and compile baseline data as assigned.
  • Support people so they may be included and participate in their community.
  • Work collaboratively with others to ensure the person's health and wellbeing.
  • Ensure emergency medical procedures are followed and consult with the medical chain of command when necessary.
  • Report and document all medical concerns the person may have to the appropriate personnel.
  • Transport people to health/medical appointments and activities as assigned.
  • Maintain on-going communication with the Intake/Outreach Coordinator
  • Address complex needs such as homelessness, hoarding, food insecurity, or infestations with compassion, effectiveness and a non-judgmental manner.
  • Provides short-term case management services in compliance with OMH and Outreach contracts.
  • The position involves addressing the immediate needs of the individual and fulfilling the responsibilities in the IPOP.
Program/Service Management
  • Maintain an environment which encourages and facilitates optimal personal development.
  • Complete documentation, filing and general office maintenance and other routine operational reports, charge reports and any other paperwork as assigned.
  • Assist in identifying new opportunities to enhance service quality and increase program offerings that provide quality of life choices.
  • Maintain knowledge of and effective use of agency software programs, including the Electronic Health Record and other documentation platforms.
  • Provide education and information on OPWDD, the eligibility process, HCBS waiver, Care Coordination Organization and Arc services to people with IDD, families, and organizations such as school, local civic organizations, law enforcement agencies, and other community organizations.
  • Collaborate with the supervisor and development office to identify and attend outreach activities, create goal-oriented presentations for the Outreach contract, and maintain a resource list of community organizations and partners to assist people supported through assigned contracts.
  • Develop expertise in the identified community resources and partnerships to effectively support each person's needs.
  • Collaborate with the supervisor to develop safety plans around service provision as needed.
Contacts with Others
  • Develop and maintain positive customer relationships both internal and external. This includes family, local businesses, and internal and external service providers.
  • Ensure that internal and external written and verbal communications regarding the agency are positive and reflect our mission, vision and values and respect the person's right to privacy and confidentiality.
  • Address all concerns promptly and professionally, including discussions on sensitive topics, while demonstrating the ability to clearly explain complex subjects.
  • Represent the agency professionally at community meetings, including Single Point of Access (SPOA) meetings as assigned.
Physical Plant and Safety
  • Maintain knowledge of health and safety plans and ensure plans are followed
  • Maintain knowledge of fire evacuation plan and ensure plan is followed.
  • Maintain an environment that encourages and facilitates optimal personal development.
  • Practice standard precautions and other appropriate infection control practices.
  • Ensure site vehicles are maintained, if applicable.
Additional Functions
  • Participate in agency committees, initiatives, projects and trainings.
  • Assist in the scheduling and provision of outreach/advocacy activities, ICS and other services.
  • Assist with coverage within the Division and Agency, as assigned.
  • May frequently be required to use your private vehicle to transport the people we support.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Exhibits conscientiousness, dedication, self-discipline, and a sense of responsibility.
  • Exercise significant judgment to work independently, take initiative, and effectively prioritize multiple tasks while maintaining attention to detail and meeting deadlines in a fast-paced, high-pressure environment.
  • Ability to exercise discretion in handling confidential information and matters of significance. This includes medical, financial and personal information of associates and people receiving support.
  • Demonstrates knowledge of introductory computer skills.
  • Excellent written and verbal communication skills, including public speaking ability.
  • Ability to operate wheelchair vans, if applicable.
  • Ability to interact with multiple service and community organizations in an effective manner.
EDUCATION AND EXPERIENCE:
  • Associate Degree and 1 years' experience or high school diploma/GED with 3 years' experience in the IDD field.
  • CPR/First Aid/AED and SCIP Certification within the first six months of hire.
  • Medication Administration Certification for Residential and site-based program, as required.
  • Valid driver's license for the state in which the person resides
