The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization dedicated to advancing the health and well-being of all New Yorkers. In partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives to advance community health throughout the city.
PROGRAM OVERVIEW
The Bureau of Equitable Health Systems (BEHS) is part of the Division for Center for Health Equity and Community Wellness at DOHMH. Our division strengthens the Department's ability to partner with the NYC health care system. The Community Healthcare Support Team (CHST) within BEHS operates NYC REACH, helping New York City providers implement new health care delivery models and quality initiatives.
POSITION OVERVIEW
BEHS is seeking a Program Coordinator to support program staff and partners in implementing best practices for quality improvement initiatives in primary care. The Program Coordinator will assist in tracking team and grant deliverables for the Cardiovascular Health Innovation Program (CHIP), aimed at addressing health disparities in cardiovascular disease in high-risk neighborhoods across New York City.
RESPONSIBILITIES
QUALIFICATIONS
• 2+ years of relevant experience• Ability to work collaboratively with physicians, staff, and external organizations• Excellent time management, attention to detail, and organizational skills• Excellent verbal, written, presentation, remote communication, and group facilitation skills• Working knowledge of Microsoft Word, Excel, PowerPoint, and Outlook• Knowledge about chronic disease prevention/management is a plus

There is potential for this position to transition to DOHMH, and candidates must meet DOHMH eligibility requirements, including NYC residency.

You must live in the New York City Tri-state area (NY, NJ, CT) to be considered for a position at FPHNYC.
