Base pay range

$75,000.00/yr - $80,000.00/yr

We are seeking a dedicated Program Coordinator to join our Brooklyn Transitions Family Treatment/Rehabilitation (FT/R) Program. This specialized, home-based prevention program supports families facing child welfare challenges due to substance abuse and mental health concerns, utilizing the Solution-Based Casework (SBC) model. The Program Coordinator plays a key leadership role in ensuring the program's success by overseeing operations, staff management, and program implementation in collaboration with the Program Director.

Key Responsibilities

Leadership & Team Support

  • Serve as a program and agency culture carrier, promoting the FT/R and SBC models.
  • Uphold the agency's mission, vision, and values, ensuring positive staff morale.
  • Effectively communicate with staff and external stakeholders.
  • Ensure smooth and efficient program operations.
  • Maintain high expectations for performance excellence.

Program Operations & Oversight

  • Recruit, hire, and support high-performing staff.
  • Implement and manage agency, funder, and model policies to exceed expectations.
  • Prepare and report monthly statistical and tracking data.
  • Coordinate intake processes to maximize program capacity.
  • Supervise Family Team Conference Facilitator to ensure timely and effective conferences.
  • Facilitate Clinical Diagnostic Team (CDT) meetings to support strong clinical decision-making.
  • Provide clinical consultation regarding substance abuse and mental health concerns.
  • Lead or participate in family sessions, workshops, and program events when needed.
Qualifications
  • Licensed in NYS: LMSW, LCSW, LMHC, LCAT, or LMFT (Required).
  • 4+ years of experience in child welfare, mental health, or substance abuse settings.
  • Strong commitment to cultural humility and anti-racist practices in child welfare.
  • Experience working with individuals facing substance abuse and mental health challenges.
  • Excellent group facilitation and problem-solving skills.
  • Strong ability to multi-task and manage administrative details.
