Job Description Program Director; 4224-201-N 91-14 Merrick Blvd, Queens, NY 11432, USA Req #3585 For over 117 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. STATEMENT OF THE JOB: The HLPS Program Director leads/manages a team of several staff to develop and execute housing navigation and placement services including follow-up post placement for clients referred by NYC Health + Hospitals (H+H). The Program Director plays a key role in active daily monitoring and reporting to ensure all activities are met on time, new potential units are identified, and casework and reporting documentation is clear. The Program Director will work with triaging difficult cases, trouble-shooting incidents, and anticipating challenges.As is necessary, the Program Director will support the client and their staff to coordinate direct services, service referrals, or evaluations with other points of contact to support the client, including shelter/safe haven housing specialist, H + H care team, etc.The Program Director is expected to be an active member of the CBC Consortium. This consortium works together on meeting contractual requirements, develops best practices, and provides mutual support. The Program Director is accountable for the agency s responsible contract adherence through regular review of metrics and deliverables.* Fluent in contractual requirements, electronic housing tracking system, and budget management.* Oversee clients are efficiently matched to housing opportunities identified through the CBC and HLPS Team, partners and other citywide stakeholders.* Supervise housing navigation team (case manager, housing specialist, peer specialist, apartment readiness worker).* Monitor the complete process from referral, assessment, housing acceptance, rent-up through move-in, and post placement.* Implement vouchers or lease renewals system as needed.* Ensure housing placement process and operational targets are achieved on weekly, monthly and quarterly basis. Identify barriers, trends, and implement rapid cycle improvements with HLPS partners.* Facilitate staff meetings case conferences, group supervision, and identify, promote, and attend professional development activities such as in-service trainings, seminars, webinars, and conferences.* Participate in and contribute to CBC Consortium with other Partner Team Leaders and H + H representatives to monitor project implementation (e.g., referrals received, patient needs, outcomes documented, etc.) develop standards of care, and document best practices.* Work closely with maintenance, finance and program services departments to track spending on the HLPS grant on a monthly basis. Regularly submit invoicing to CBC and track purchase orders and payments.* Support an environment that embeds diversity, equity, inclusion, and accessibility (DEIA)* Perform all job-related tasks as required. QUALIFICATIONS: * Master s Degree in a Human Service-related field from a nationally accredited .* Institution Minimum of 2 years supervisory experience in the BH field.* Good working knowledge of behavioral health, supportive and affordable housing programs, and government regulations.* Experience working with homeless individuals and people living with health and/or mental health issues and substance use a plus.* Highly organized, with an ability to prioritize time-sensitive assignments.* Demonstrated commitment to quality and data-driven reporting as applicable.* Action-oriented, entrepreneurial, flexible, and innovative approach to supervising staff, supporting clients and collaborating with colleagues and programs with similar needs.* Regularly required to talk, hear, walk, stand, & sit.* Able to lift up to 10 pounds.* Able to operate a computer keyboard, mouse, office equipment, and write reports.* Able to read printed materials and computer screens.* Able to travel to multiple locations as needed.* Able to work flexible hours and days - including weekends/evenings/holidays according to needs of program.
