Under general direction of the Vice President of Resident Services- Affordable Housing, the Program Manager (PM), provides oversight of the development and implementation of CalAIM (California Advancing and Innovating Medi-Cal) Program-Community Supports, and ensures that programs and resources are aligned with HumanGood's goals and the CalAIM regulations at several of its affordable housing communities, and performs related duties as assigned. In this role you will be responsible for overseeing CalAIM in areas such as program design, development and implementation, including managing partner relations, program outreach, client authorizations, client support and case management, claims billing and collections, and other support services in partnership with designated service providers. You will also be responsible for developing and implementing new strategies in collaboration with other agencies; training HumanGood team members; performing research, program evaluation, documentation, tracking, reporting, fiscal management and program expansion efforts related to CalAIM. This hire will ensure all work focuses on strategic fulfillment of CalAIM strategies and leads the plan to support the health and independence of residents living in the communities.Program Development and Implementation:
- Create, lead, execute the development and implementation of CalAIM Program by conducting feasibility analysis to draft program development plans, outreach strategies plan, CalAIM team onboarding and training and monitoring CalAIM program deliverables.
- Develop, implement, and maintain HumanGood's policies and procedures to ensure continuous compliance with program regulations.
- Develop and deliver CalAIM operations training, review of Policy and Procedures and establish standard operating procedures to ensure high quality service delivery and resident outcomes.
- Gain deeper understanding of resident populations at the designated CalAIM communities to enroll them into CalAIM programs and services in a timely manner.
- Build a close collaborative working relationship with community housing administrators and resident services coordinators to collaborate effectively and achieve CalAIM objectives and resident goals.
- Coordinate cross-functional strategies for optimum impact and scale.
- Stay updated on relevant legislation and regulations impacting CalAIM Community Supports and engage with the CalAIM PATH collaboratives on a routine basis.
- Build and strengthen partnerships with HumanGood departments, local service providers, and the CalAIM provider network.
- Develop and manage program budgets.
- Track and oversee reporting for awarded PATH grant funds and other HumanGood stakeholders.
- Deliver clear and concise oral and written reports on CalAIM services to designated HumanGood representatives and other stakeholders.
Client Services and Support:
- Deliver comprehensive case management services to resident/clients.
- Create and review individualized service plans, identify client needs, and make timely referrals to ECM and other Community Supports providers.
- Document and review all services and encounters in the SpriteHealth platform.
- Organize and securely store CalAIM-related documents and files in the Teams channel.
- Actively participate in CalAIM service provider meetings.
- Build and maintain strong relationships with service providers to ensure timely delivery of necessary services and program compliance.
- Proactively identify and resolve key issues with service providers
- Develop a thorough understanding of SpriteHealth platform and create documentation policies and procedures manual to assist with team training.
Administrative and Technical:
- Monitor sFTP files for timely authorizations, approvals, and receivables.
- Represents HumanGood at industry conferences, workshops, and meetings.
To be successful in this role, you would have:
- Bachelor's Degree in Gerontology, Social Work, Public Health, Healthcare Administration, or related field and 4 years of experience serving older adults or a Master's Degree in the same fields with 2 years of experience serving older adults.
- 2+ years of experience in program development; working knowledge of social, economic, and political issues relevant to low-income communities; and trends in the field of healthcare and social services;
- 2+ years of experience working with either of these CalAIM Populations of Focus such as Individuals experiencing homelessness, OR individuals at risk for avoidable hospital or Emergency Department utilization, OR individuals with serious mental health and/or SUD needs, OR Adults living in the community and at risk for long term care institutionalization
- Experience with administration of various assessment tools, creating, implementing and monitoring care plans for residents and care coordination with other providers
- Current knowledge of Federal, State and local laws, regulatory codes, ordinances, procedures relevant to social services programs, such as those designed to improve/provide housing, education, employment, or socio-economic status
- Principles and practices of performing needs assessments; program development, implementation, management and evaluation
- Conflict resolution and problem-solving techniques
- Experience with community outreach, advocacy and public education
- Principles and practices of staff management, supervision and training
- Strong leadership and supervisory skills
- Strong oral, written, presentation, interpersonal and diplomacy skills

Location: This position will be hybrid: 3-4 days/week scheduled onsite visits across (3) properties in the South San Francsico/Daly City region, and 1-2 days remote. Schedule is subject to change as the program, staffing structure and duties of the position and program evolve.Compensation: 82-92K depending on experience + performance-based bonus
