POSITION OVERVIEW:

The Program Manager provides oversight and management of program activities related to care coordination from recruitment, testing, and treatment, overseeing the process from intake through treatment adherence. The Program Manager will work closely with the Senior Administrator to inform the community of services and establish referral relationships with community partners.

This position pays: $32.96 per hour.

KEY ESSENTIAL FUNCTIONS:

  • Develop, implement, and monitor a client outreach and recruitment plan responsive to the needs of the community population.
  • Ensure screening process is conducted within Article 28 for patients and residential program attendees.
  • Lead initiatives related to care coordination, overseeing the process from intake to assessment to treatment adherence as part of a care coordination team.
  • Serve as the on-site primary contact for administrative and medical document retention and dissemination to coordinate care services, including confirmation of identification and assistance with procuring documents to begin or continue care.
  • Lead the multidisciplinary care team to identify effective systems-level needs to improve quality and efficacy of services.
  • Structure weekly case conference meetings for the multidisciplinary care team.
  • Ensure staff participates in health literacy and effective communication training and related trainings as per workplan.
  • Conduct consistent supervision with the team for support and addressing programmatic tasks.
  • Responsible for required reporting, including a monthly narrative that describes monthly testing, enrollments, highlights, and challenges.
  • Oversee program monthly data reporting (via AIRS) for accuracy prior to submission.
  • Complete and submit the annual budget.
  • Oversee and ensure the program is providing assistance to participants in scheduling and keeping physician appointments, supervising the linkage to care specialist and patient navigator to arrange escort/transportation to appointments if needed.
  • Implement onsite educational sessions and support groups for individuals interested in hepatitis C treatment and treatment management.
  • Ensure education to participants regarding injection drug use, personalized risk assessment, overdose prevention, HCV prevention, treatment and post care, and HIV transmission and prevention.
  • Develop educational messaging regarding testing, treatment, re-infection, and wellness for participants in pre and post treatment care services.
  • Manage and provide crisis intervention that may arise with participants.
  • Participate in team meetings, staff meetings, and professional development opportunities as directed.
  • Provide audit-worthy documentation for all services provided to participants in both the Electronic Health Records (EHR) and Access Database. Collaborate with the Senior Administrator and QI Coordinator on program reports and service utilization.
  • REQUIREMENTS:

  • High School Diploma required; BA preferred.
  • Bilingual in English and Spanish required.
  • 2 to 3 years of experience working with marginalized populations (people who use drugs, sex workers, mentally ill, homeless, and/or formerly incarcerated).
  • Pragmatic and competent problem-solving and conflict resolution skills.
  • Able to conduct street-based and park-based outreach; physically able to climb stairs.
  • Experience working with the homeless population.
  • Ability to communicate effectively both verbally and in writing.
  • Must have computer proficiency including but not limited to Microsoft Office and web search engines.
  • Ability to work with diverse communities (gender, ethnic, generational, lingual) as well as enthusiasm for working with individuals who are current substance users.
  • Ability to build relationships with clients, service providers, and other programs within the community.
  • Ability to handle confidential information with discretion and integrity.
