Under the direction of the Director of Provider Practice, the School-based Health Center Care Coordinator assists in the recruitment of Mile Square School Based Health Center participants, coordinates screenings, referrals and linkages with community stakeholders, and attends relevant meetings or focus groups. Collaborates with partner organizations to address the issues affecting the community. Identifies and/or develops health/wellness outreach programming to be integrated with those programs currently offered by Mile Square School Based Health Centers. Solicits community resources for programming and assumes responsibility for publicizing/marketing all outreach efforts.

The School-Based Health Care Coordinator is essential in navigating and connecting students to health and social resources within the school, school-based health center (SBHC), and community. The coordinator is responsible for collaborating with an interdisciplinary team to develop tailored plans that support the students.

Duties & Responsibilities

* Under the direction of clinical staff using standard protocols, assesses, refers and navigates new and established patients for completion of prescribed screening, treatment and referral activities.

* Provides educational information and outreach regarding programs, associated requirements, and other opportunities to community residents and the public.

* Serves as a resource person for participants in the program: develop referral relationships with healthcare providers and other community-based providers.

* Manages and coordinates outreach events in order to attract new families to the Mile Square School Based Health Centers.

* Coordinates professional presentations to community groups on educational topics.

* Serves as a liaison between the institution or agency and the individuals served; refer patients support services when needed and follow up on referrals' and assist participants in utilizing referral services.

* Motivates participants to keep appointments, follow-ups on referrals and participates in health centers and Community meetings.

* Works with staff and program participants to assess their community for strengths, problems, and concerns relative to healthcare (e.g. conducting surveys, interviewing, etc.) Assesses, identifies, and assists in establishing services needed within the school community.

* Markets and promotes services and programs on a scheduled basis.

* Strengthens existing relationships and cultivates new ones through working with existing community stakeholders and new community stakeholders alike.

* Lead the development of an Outreach Plan for targeted communities, with input from the OCEAN-HP and Mile Square School Based Health Center leadership. Implement the plan within the timelines established.

* Complete all required reports to document activities and provide data to measure impact.

* Works as part of the SBHC team with students, parents/guardians, school health staff, and community organizations to implement a written care plan that includes a health summary and action plan.

* Supports the coordination of care management through scheduling, referrals, and follow-ups.

* Tracks the progress and completion of assessments and ordered tests and reports to clinical team as appropriate.

* Helps students and parents/guardians navigate multiple systems across health, educational, and community settings (e.g., consents, privacy and data protocols, and health insurance).

* Uses health information technology and data to proactively track the effective delivery and continuous monitoring of care coordination.

* Assists in data collection and utilization efforts, mainly focusing on using electronic records or other health technology to collect data that support the health and education connection.

* Recognizes students and families' cultural and language assets and works to actively address and support their needs.

* Builds and maintains trusting relationships with students and their families, assessing strengths and incorporating them into the care plan.

* Communicate effectively to align with the expectations of students and their families.

* Ensures transparent data sharing under appropriate permissions with students, families, school staff, and other relevant stakeholders (e.g., participating in relevant school meetings, such as Individualized Education Plan (IEP) meetings and attendance meetings, where students' health is appropriate.)

Minimum Qualifications Required:

* Any one or combination totaling three (3) years (36 months) from the categories below:

* Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:

* 30 semester hours equals one (1) year (12 months)

* Associate's Degree (60 semester hours) equals eighteen months (18 months)

* 90 semester hours equals two (2) years (24 months)

* Bachelor's Degree (120 semester hours) equals three (3) years (36 months)

* Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.
