Job Description :
KEY RESPONSIBILITIES The Program Specialist is responsible for upholding Abound Health, LLC's mission, vision and values by providing support, services, education and compassion to Clients receiving Pennsylvania Waiver Base Funded Services.   • Review assigned client's service/grid notes within a timely manner
  • Review all verification/documentation of services for appropriate signatures and documentation per the ODP Records Management and Documentation Manual
  • Ensure assigned direct care staff records are current
  • Ensure assigned client clinical records are current
  • Conduct and document clinical and employee supervisory visits to ensure Abound Health Policies/Procedures are followed
  • Conduct annual performance evaluations, direct care staff development plans, disciplinary meetings/documentation with direct care staff
  • Participate in required staff meetings
  • Participate and advocate for the client in the client's ISP meetings
  • Inform plan members of the option to decline the ISP Review documentation as required
  • Develop individualized short-term goals, task analysis and interventions for assigned clients
  • Complete client's annual skills and preference assessments (as applicable)
  • Ensure all RDSPs are completed (as applicable)
  • Ensure client specific trainings are completed for every client/staff relationship
  • Enter goals into OnTarget
  • Review weekly billing to ensure hours are used
  • Complete monthly/ quarterly progress reports
  • Recommend a revision to a service or outcome in the ISP as needed
  • Review and update task analysis per client's goal frequency
  • Ensure staffing patterns are covered and back up staff is available
  • Resolve and document client/guardian grievances and report to Supervisor
  • Complete significant event and incident reports within 24 hours (client and employee/contractor)
  • Schedule direct care staff to provide direct care. Hours must comply with service authorizations and waiver limitations
  • Assist the clients and families with finding and procuring appropriate resources and services in the community
  • Represent Abound Health at Community Events
  • Perform daily activities per company's needs
  • Any other duties assigned by the CEO, COO, CCO, Program Director

  • Provide privacy and confidentiality of all participants' records.
  • Assure participants are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor.
  • Treat each participant with dignity and respect.
  • Develop and demonstrate a thorough understanding of Abound Health's philosophy and purpose.
  • Provide a positive atmosphere which facilitates growth and learning of coping skills.
  • Perform duties as requested by Supervisor

  • Understanding of requirements of third-party billing and Medicaid billing
  • Maintain Therapeutic working relationship with Clients, Family Members, Team Members
Credentialing Must be credentialed as a Program Specialist with either:
  • A Master's Degree or above from an accredited college or university and 1 year of work experience working directly with persons with disabilities.
  • A Bachelor's Degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities
  • An Associate's Degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities.
  • Requirements
    • Program Specialist Qualified
    • At least 18 years of age
    • Valid Driver License
    • Safe Driving Record
    • No substantiated findings of abuse and/or neglect
    • Criminal, PA State Police, FBI (if required), SAM, MEDI Check, Sex Offender, LEIE, PA Child Abuse History (if required), Driving Record checks clear of health and safety risks.
    • Reliable and safe transportation
    • Current Auto Insurance
