Responsible for outreach activities, coordination of services of families, and assistance with specific health care, social, educational and employmentactivities and referrals. Organize schedule with the supervisor to accommodate needs of families and program requirements. Work assignments are directed by the Senior Public Health Aide. Does related work as required.MINIMUM QUALIFICATIONS:Possession of a high school diploma or general equivalency diploma.SPECIAL REQUIREMENT: At time of appointment and throughout employment:1) Valid NYS driver's license.2) Ability to work flexible hours including evening.3) Successful clearance from the State Central Registry in the New York State Office of Children and Family Services (Background investigation is conducted by the appointing authority).Minimum Salary: 34,634 Maximum Salary: 41756 Salary Unit: Yearly
