Salary : $65,393.00 - $98,089.00 Annually Location : Brainerd, MN Job Type: Full-Time Job Number: 2025-00002 Department: Community Services Opening Date: 01/07/2025 Closing Date: 2/4/2025 4:30 PM Central Position Objective Join a caring team to perform responsibilities of public health nursing through assessment, plan development, implementation and evaluation activities aimed to improve the level of health across the County. Work is performed in an office, institution, community, home, school or community-based setting. Essential Duties & Responsibilities
  • Performs independent health assessments of individuals, families and communities by evaluating their physical, mental, social, and environmental health status; provides nursing and community health intervention based on assessment of need and current community resources.
  • Provides case management for clients by developing care plans, consulting with other health care providers such as physicians and social workers, referring clients to other health care resources, participating in team meetings, coordinating health care services, and instructing clients and families on methods of positive health maintenance; initiates and conducts interdisciplinary conferences.
  • Provides leadership in projects related to the development, implementation, evaluating and maintenance of public health programs.
  • Develops, implements, coordinates, and evaluates interventions in order to meet grant and program objectives.
  • Consults, informs, and educates different audiences via public speaking, social marketing, mass media campaigns, and the development of educational materials.
  • Represents the Department and the County both internally and externally in state, county, local communities, work groups, medical providers, and task forces in order to assure at-risk populations receive program services.
  • Serves on internal and external work groups, task forces, and advisory committees.
  • Researches and makes recommendations to senior leadership for advocacy initiatives for public health programs and resources.
  • Assists in evaluation of program outcomes including interpretations of reports to recommend on-going planning and establishment of intervention priorities. Proposes responses to identified needs using various methodologies.
  • Recommends program changes to supervisor by researching health trends and community needs; by interpreting the results of data collection; by identifying resources; and by preparing reports and grant requests.
  • Initiates contacts with potential grant sources and drafts recommended proposals for funding; manages grants by maintaining data related to activities and drafting periodic reports to grantors as required.
  • Performs public health emergency response functions as trained and assigned within the department's emergency response plan.
  • Participates in and takes direction to conduct disease investigation, and activate appropriate control and prevention plans as assigned.
  • Assists to identify the gaps in the quality and accessibility of health care service for Crow Wing County residents and make recommendations to management and policy makers for change.
  • Represent the agency and collaborate with community groups to promote and improve the health of the whole community.
  • Intervene in acute communicable diseases and implement effective public health practices.
  • Maintain proper documentation of records and reports according to standards of practice.
  • Attend required and necessary meetings and training. Maintain a high level of professionalism.

Minimum Qualifications
  • Bachelor's degree or higher
  • Certification as a Public Health Nurse (PHN) in the State of Minnesota or ability to obtain within 3 months of hire.
  • Current RN registration with MN Board of Nursing
  • CPR Certification
  • Valid state issued driver's license and vehicle
Desirable Qualifications
  • Previous training and/or experience as a RN or PHN In Family Home Visiting
  • Experience working in Nightingale Notes Omaha Documentation System
  • Thorough skill creating and maintaining adequate records
  • Knowledge of data privacy laws, Minnesota Statutes Chapter 13, Minnesota Rules of civil procedure and HIPPA.
  • Knowledge of local, state and federal laws and regulations relating to health and social services sufficient to understand the impact on the client and service delivery parameters.
  • Ability to foster and maintain effective working relationships with staff, clients, agencies and community groups and public.
  • Ability to organize, prioritize and work independently.
  • Knowledge of office and medical equipment.
  • Ability to communicate effectively orally and in writing, provide testimony and present to groups and/or individuals.
  • Trained in Incident Command: NIMS IS700, IS100, and IS200
  • Knowledge of Public Health Practice Standards, Standards of Nursing Practice, Vulnerable Adult Act and Child Abuse/Neglect law.
  • Knowledge of community resources.
  • Ability to administer medication, vaccinations and collect laboratory samples.
  • Ability to be creative in problem solving and be assertive.
  • Strong interpersonal skills and a commitment to public health nursing.
  • Ability to be flexible in job assignments and interest in a variety of duties.

Compensation The county strives to be a market competitive employer with compensation rates that are externally competitive and internally equitable. Compensation rates are governed by the county pay matrix and applicable labor contract. Information regarding the county pay programs is available in the Human Resource Department. Paid Time Off (PTO) PTO benefit accrues at the rate of 21 days per year for the first 3 years for regular full-time staff and is pro-rated for regular part-time staff. Holidays The 12 calendar holidays that are observed by Crow Wing County are as follows: New Year's Day, Martin Luther King, President's Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Veterans' Day, Thanksgiving Day and Friday following, Christmas Eve and Christmas Day. Health Insurance Regular full-time employees and their dependents are eligible for group health insurance coverage, administered by Blue Cross Blue Shield. Eligibility begins on the first of the month following 30 days of employment. Regular part-time employees regularly scheduled 30 hours or more per week are eligible for health insurance coverage on a pro-rated basis. Dental Insurance Delta dental benefit is available to those participating in a health insurance plan. Calendar year annual maximum of $1000 per person. Life Insurance Regular full-time employees shall be provided a term life insurance policy of $50,000. Optional life insurance is available at the employee's expense. Short-Term Disability Regular full-time and part-time staff regularly scheduled at least 30 hours per week are eligible for short-term disability following 6 months of qualifying employment. Short-term disability payments begin on the first day following 14 consecutive calendar days of absence attributable to a qualifying disability. The plan provides income replacement of up to 50% of regular pay subject to a maximum limit of $1,000 per week. Long-Term Disability Regular full-time and part-time staff regularly scheduled at least 30 hours per week are eligible. Long term disability pays 50% of salary up to a maximum of $4,000 per month beginning on day 91 of total disability. The maximum benefits duration is up to the social security normal retirement date.
