Also, must have the ability to provide highly skilled professional nursing in a designated area or specialty such as community health and home visiting and have the ability to utilize the nursing process in the establishment of care for our clients.
- This position is to recruit a Part-Time Nurse Home Visitor for our Program.
- CaCoon Nurse Home Visitors help families coordinate care for their children and youth with special health needs.
- Plan and conduct shared plans of care.
- Co-case manage with a Community Health Worker to support child/families social needs.
- Work with families and the care team to identify the child's strengths and needs.
- Connect with healthcare and other community supports.
- Make sure the child's health team works well together.
- Help gather information to make health-related decisions for the child.
- Identify problems and help find solutions.
- Assess, plan, evaluate and implement general community health nursing service in assigned area of responsibility or special program.
- Make home or office visits, giving direct patient care within the scope of standing orders, explaining basic concepts of prevention of disease and promotion of mental and
- physical health; teaching and demonstrating to patients and families nursing care and assisting families in understanding health needs and methods of meeting needs.
- Plan and exchange information with physicians and other health workers for more effective care of the individual patient; take responsibility for development of community organization and planning for health services with community groups.
- Maintain records for nursing services to families and clinics, and make reports as required.
- Develop and maintain effective, harmonious and reasonable work relationships with others.
Minimum Qualifications Qualified candidate must possess a license to practice as a Registered Nurse in the State of Oregon. Possession of a valid motor vehicle operator's license and an acceptable driving record at the time of appointment is required. Experience working with children and youth with special healthcare needs in any setting preferred, not required.
