ProgramQuality AssuranceJob SummaryInvestigates incidents as assigned. Review the incident reports and determine the direction of the investigation, complete all interviews, write a detailed investigation report within regulatory time frames. Uploading investigations in the state database within the regulatory timeframes. Ability to recognize any health and safety issues and report to appropriate parties. On call responsibilities may be required.Essential Duties and Responsibilities include the following:
  • Determine how an investigation will be investigated, such as whom to interview and what documents should be collected.
  • Interview all relevant witnesses and the subject of the report if an allegation of abuse, neglect, or mistreatment. Interview all relevant witness for all incident investigations, which may possibly include witnesses or experts who are not employees of Lifetime.
  • Provide a detailed written investigation based on the factual information collected from interviews and supporting documentation.
  • Contact supervisors (QA/Program) immediately if any health and safety issues are discovered during the course of an investigation.
  • Contact supervisor (QA/Program) immediately if additional incidents/allegations are discovered during the course of the investigation to ensure the person's (s') safety.
  • Participate in Weekly Quality Assurance meeting and present any investigation concerns.
  • Provide Incident training as needed.
  • Serves as a positive role model. Maintains neat appearance, hygiene and grooming. Promotes positive behaviors and interactions with all people supported and employees.

  • QualificationsA Bachelor's Degree or relevant investigator experience in Human Services, Education, or related field. Would consider an outstanding candidate with minimum of 1 year experience in a supervisory position working with people with developmental disabilitiesSalaryCommensurate with experienceLocationRochester, NYHours40 Hours a WeekScheduleMonday-Friday 8:00 - 4:30, must have some flexibility for interviews outside regular hours.
