- Prior Quality Assurance experience in the ID/DD field, with a focus on conducting investigations and writing reports highly preferred (certified investigator is ideal)
- Associate Degree in Human Services or related field required
- A minimum of two years' experience working with individuals with intellectual or developmental disabilities required
- Excellent verbal & written communication skills including strong public speaking skills
- Strong time management skills & ability to meet/exceed deadlines
- Strong organizational skills with the ability to request and track deliverables from all departments
- Working knowledge of OPWDD & Justice Center regulations, guidelines & procedures required
- Self-guided & proficient in Microsoft Office (Word, Excel, Outlook, etc.)

Responsibilities:
- Conducts incident investigations; interviews appropriate parties, ability to make decisions & determines outcomes, prepares investigative reports thoroughly with written recommendations to the proper departments.
- Follows up with internal departments regarding Corrective Action Plans (CAPS) and updates QA tracking system regularly.
- Reviews and ensures incident reports are submitted to OPWDD and the Justice Center in a timely manner.
- Provides timely and effective incident management communication and updates via OPWDD IRMA & WISR for all New York State regional offices.
- Responsible for monitoring the 24/7 QA Hotline on a rotating basis and escalates matters of importance appropriately.
- Assist with conducting orientation and other incident reporting/incident management trainings for various levels of staff and newly hired employees -may conduct site visits to programs when necessary.
- Prioritizes job tasks to ensure timely and accurate completion of required work.
