  • Shall collaborate with coworkers while caring for individuals in the Regional Assessment Center (RAC): Acute Treatment Unit (ATU), Transitional Residential Treatment Unit (TRT), and/or Withdrawal Management Unit (WMU).
  • Shall appropriately and effectively care for, monitor, and support individuals in the RAC while coordinating services with fellow RAC Team Members and other Valley-Wide staff as necessary.
  • Shall complete all RAC tasks in an effective and timely manner, including meal coordination, daily living needs assistance, facility cleaning, phone calls, and other unit chores.
  • Shall complete all required documentation in a timely manner, consistent with agency and state guidelines.
  • Shall advocate on clients' behalf for resources and access to services.
  • Shall collaborate with clinical staff in the development and implementation of the plan of care and achievement of goals.
  • Shall provide behavioral health group and individual treatments for individuals admitted to the ATU.
  • Shall attend team meetings in order to facilitate an integrated, multidisciplinary team approach to services:
    • Individual supervision for review of recent contact with patients and review of documentation and
    • Clinical supervision meetings, as appropriate, for training and education.
  • Shall maintain confidentiality and adherence to HIPAA and 42-CFR requirements at all times.
  • Through collaboration with clinical supervisor, shall provide clinical assessment, problem identification, need for medical necessity (DSM 5 criteria/justification), evidence-based treatment modality, crisis intervention, individual/family/group therapy or other activities, case management, advocacy, and outreach/prevention and intervention.
  • For assigned caseload, shall develop and monitor treatment plans, monitor treatment progress, document significant interactions, and maintain clinical charts and other reporting data, including the following:
  • Attend supervision/training (individual and group), as scheduled;
  • Follow supervisory direction and ask questions appropriately;
  • Full completion of Treatment Plan, initial or intake, within 24 hours of admission;
  • Daily Progress Note completion by end of the shift;
  • Shall obtain/maintain QMAP certification.
  • Shall successfully complete all required new hire training by the designated deadline, as well as appropriately complete all subsequent training.
  • Shall coordinate comprehensive treatment-internally and externally-with high quality, holistic care as the expected outcome.
  • Shall appropriately incorporate Cultural Competency into the treatment setting and all applicable documentation.
  • Shall adhere to the Risk Stratification Model, providing appropriate Model-based care for all Valley-Wide clients.
  • Shall appropriately assist with crisis services and client transports upon request, including outside regular business hours.
QualificationsKnowledge, skills and ability:
  • English (speaking, reading, and writing); communication, technological, analytical, problem solving, and organizational skills, including substantial thought processes, concentration, reasoning, and memorization; adequate knowledge and skill level in the field of behavioral health assistance within a treatment facility
Education or Formal Training: High School Diploma or EquivalentExperience: 2 years' experience in healthcare, human services, customer service, or related fieldQualities and Characteristics:
  • Demonstrates accuracy and high attention to detail
  • Ability to multi-task and above average math skills
  • Proficient in use of office equipment i.e. 10-key, e-mail, fax machine
  • Computer skills with an emphasis on Excel and Microsoft programs, experience in computerized accounting software preferred.
