Position Summary:

This is responsible for providing comprehensive career and matriculation services and engaging with both incarcerated students and individuals recently released from incarceration. This role focuses on supporting reentry efforts by providing ongoing support and guidance as they navigate the reentry process and access essential services for a successful transition. Engage students using an Integrated Case Management approach to assess their readiness for programming, identify strengths and challenges, and collaboratively develop a comprehensive Career Pathway Reentry Plans. The Case Coach will work closely with jails, prisons, community organizations, workforce training providers, and behavioral health services to ensure a holistic and seamless reentry process.

This position requires strong relationship-building skills, the ability to navigate reentry resources, and the capacity to provide individualized support to participants as they transition back into the community.Tips: Provide a summary of the role, what success in the position looks like, and how this role fits into the organization overall.

Job Responsibilities

Case Management & Reentry Support

  • Manage a caseload of 35-40 active students, all of whom are incarcerated students and individuals recently released from incarceration.
  • Provide in-depth, wrap-around case management services tailored to individual needs.
  • Conduct initial screenings to determine service levels and support needs in the prison.
  • Develop individualized reentry case plans to address barriers such as employment, housing, education, and behavioral health services.
  • Facilitate individual case management sessions at regularly scheduled intervals.
  • Maintain detailed documentation of student interactions, including time, date, type of contact, outcomes, and next steps.
  • Provide or broker services to ensure holistic, wrap-around support for clients.
  • Conduct frequent follow-ups via phone, prison visits, home visits, school, or community locations.
  • Advocate for students' employment and educational opportunities.

Coordination & Community Engagement

  • Collaborate with program staff in jails, prisons, and community organizations to monitor client progress.
  • Coordinate with external agencies, vocational training providers, literacy programs, behavioral health organizations, and benefits coordinators.
  • Assist clients in obtaining identification documents and applying for reentry benefits and services.
  • Work closely with faith-based groups, government agencies, and workforce development programs to facilitate a smooth reintegration process.
  • Attend agency meetings, community forums, and conferences to represent the program and strengthen partnerships.

Career Pathway Development

  • Develop a Comprehensive Career Pathway Plan for each participant, including:
  • Educational attainment goals
  • Vocational skill training
  • Career readiness preparation
  • Provide employment-focused guidance, connecting participants with job opportunities, resume-building workshops, and professional development resources.

Program Oversight & Reporting

  • Provide regular updates to the Program Manager.
  • Work closely with program funders for data collection, reporting, and compliance.
  • Assist with program monitoring and evaluation, ensuring compliance with grant and funder requirements.
  • Contribute to program development, policy recommendations, and continuous improvement strategies.

Position Requirements

  • Ability to work effectively under deadlines and within budget constraints.
  • Strong ability to work independently while also collaborating in a team environment.
  • Demonstrated ability to manage multiple tasks and meet deadlines.
  • Excellent public relations, community engagement, mediation, and conflict resolution skills.
  • Knowledge of the Philadelphia and Harrisburg Justice Systems and reentry resources.
  • Ability to maintain confidentiality with sensitive client information.
  • Strong administrative, organizational, and case management skills.
  • Excellent verbal and written communication skills.
  • Strong computer literacy, including proficiency in Microsoft Office (Word, Excel, Outlook) and database management.
  • Ability to engage diverse groups of individuals, including justice-involved youth, community partners, and public officials.

Minimum Qualifications

  • Bachelor's degree in social work (BSW), Psychology, Counseling, Criminology, or a related social/behavioral science field.
  • Master's degree in social work (MSW) is a plus.
  • 2-3 years of experience providing case management services, preferably in reentry, workforce development, or social services.
  • Knowledge of the criminal justice system and experience working with ex-offenders is preferred.
  • Ability to pass criminal background checks (State, Local, and Federal).
  • Strong interpersonal skills with a passion for criminal justice reform and social services.
  • Valid driver's license and reliable transportation.
